Affidavit of Eligibility, Release of Liability and Prize Acceptance Form
This Version is Effective: 29 August 2020
This affidavit, when signed and returned, will serve as a certification of eligibility, release of liability release and prize acceptance for the person listed below, who has been notified that he or she is the winner of the prize described in the attached rules (the “Prize”).
1. I certify that I am a legal resident in the United States of America or in the District of Columbia who is at least eighteen (18) years of age or older.
2.I certify that I have read the ‘SheFvckinBad Official Giveaway Terms and Conditions’ regarding this Giveaway and agree to the terms and conditions therein. (See Below)
3.I certify that neither I nor any relative of mine (mother, father, sister, brother) nor anyone living in my household is employed by SheFvckinBad or any of its affiliates.
4.I hereby grant to SheFvckinBad permission to use my name in any related advertising or promotional campaigns, as SheFvckinBad may deem appropriate from time to time, all without additional compensation to me.
5.I hereby release SheFvckinBad, and their respective employees, officers, directors, independent contractors, agents, representatives, affiliates, and prize sponsors (“RELEASEES”) from any harm, claims, damages, losses, liabilities, injury, cost or expense whatsoever with respect to or in connection with the Giveaway or the Prize.
6.I understand that my failure to return to SheFvckinBad a signed copy of this Affidavit within 10 business days after being notified of winning the Prize may result in my forfeiture of such Prize.
7.I accept sole responsibility as a winner of a Prize under the Giveaway. I understand that I am solely responsible for the payment of any applicable federal, state, local and other taxes imposed as a result of winning a Prize, in accordance with all applicable laws and regulations, and that the value of the prize will be reported to the IRS.
8.I acknowledge that this is a complete RELEASE and DISCHARGE of any and all claims against the RELEASEES, and that no action will be taken by or on behalf of me with respect to any such claims, it being understood that this release shall be binding upon my heirs, executors and administrators.
